LOS ANGELES – The National Council on Aging says nearly 80% of seniors have two or more chronic conditions, and in many cases those conditions can make it difficult for a person to leave their home for medical care. L.A. Care Health Plan, the nation’s largest publicly operated health plan, is proud to announce a new collaboration with Welcome Health, an advanced primary care provider for Medicare-eligible seniors, which offers a flexible care model that includes clinic-based, virtual, and in-home visits for those who face barriers to accessing traditional care settings.
“There are a variety of obstacles that keep many seniors from going to their primary care provider for appointments – they might have mobility limitations, visual impairments, or other constraints due to chronic illnesses,” said Sameer Amin, MD, L.A. Care Chief Medical Officer. “As a health plan committed to advancing health equity, we want to ensure that every one of our members has access to quality care, and for many seniors, that means care in their home.”
Founded on the principles of geriatric medicine, Welcome Health specializes in the needs of older adults and provides personalized, comprehensive care designed to help people stay healthy and independent. Care is delivered in the setting that best meets each patient’s needs – whether in the clinic, at home, or virtually – providing both convenience and flexibility.
Welcome Health will launch this collaboration as the primary care provider for eligible L.A. Care Medicare Plus (HMO D-SNP) members who select Welcome Health as their primary care provider. These members, who are eligible for both Medicare and Medi-Cal, often benefit from more adaptable, person-centered care designed to proactively manage their health and support aging in place, thereby greatly improving quality of life.

Welcome Health’s interdisciplinary care model is led by a team of physicians and advanced practice providers who follow clinical guidelines and care practices tailored specifically to the needs of older adults. They are supported by care coordinators and patient access coordinators who help ensure timely follow-up, smooth transitions, and responsive support throughout the care journey. In addition to in-person visits, Welcome Health offers 24/7 on-call support and virtual care to ensure members have access to care when and where they need it.
