Community Transition Services/Nursing Facility Transition to a Home

Community Transition Services/Nursing Facility Transition to a Home helps individuals to live in the community and avoid further institutionalization. 

Community Transition Services/Nursing Facility Transition to a Home are non-recurring set-up expenses for individuals who are transitioning from a nursing home to a home setting in a private residence where the Member is responsible for paying for their own living expenses.

Eligibility and Exclusion Criteria

To be eligible for Community Transition Services/Nursing Facility Transition to a Home, a Member must:

  1. Be currently receiving medically necessary nursing home level of care and, in lieu of remaining in the nursing home setting, is choosing to transition to a home and continue to receive medically necessary services; and 
     
  2. Have lived in a nursing home and/or Medical Respite setting for 60 days or longer; and 
     
  3. Want to move back into the community; and 
     
  4. Be able to reside safely in the community with appropriate and cost-effective supports and services.
