Community Health Services

L.A. Care provides Community Supports, medically appropriate and cost-effective services and settings, to address Members’ health-related social needs. Community Supports are intended to help Members lead healthier lives and avoid higher, more costly levels of care.

Community Supports may be available to you under your Individualized Care Plan. You do not have to pay for Community Supports, and they do not replace the benefits you already receive under Medi-Cal. If you would like to learn more about which Community Supports may be available to you, please call 1-888-839-9909 (TTY 711) or talk to your doctor.
