Complete a Comprehensive Medication Review Before End of the Year

providers talking to patient about medicationMedication Therapy Management (MTM) services, including a Comprehensive Medication Review (CMR), is available free of charge to qualifying* Cal MediConnect (CMC) members. For 2022, L.A. Care MTM program services are provided by licensed clinicians from the Navitus Clinical Engagement Center (CEC).

L.A. Care is urging all eligible members to complete a CMR before the year end. If you see a CMC member in a clinical visit who may be eligible for the MTM program, but who has not completed a CMR, please advise them to call 1.213.584.2028 or call the toll-free number 1.888.913.7885 immediately. 

The Centers for Disease Control and Prevention (CDC) has endorsed MTM services, stating that “MTM has been shown to be effective for lowering systolic and diastolic blood pressure; lowering LDL cholesterol and other health indicators (e.g., glycosylated A1C, HbA1c); increasing patient knowledge; improving patient quality of life and medication adherence; and improving the safe and effective use of medications.”1 

You may receive a fax from L.A Care, in conjunction with Navitus CEC, for members under your care who were identified as potentially having cognitive impairment and who may benefit from our MTM service. Should you receive this fax, please verify if the patient is cognitively impaired along with the diagnosis that is associated with this verification.

If the patient does have cognitive impairment, we ask for your cooperation in returning the completed fax, a current medication list and an associated diagnosis list to Navitus CEC at 1.716.803.6708 at your earliest convenience.
