Eligible members will be outreached to participate in Care Management. Engaged members can expect the following comprehensive care management services:
- Outreach and engagement
- Comprehensive assessment
- Individualized Care Plan (ICP)
- Interdisciplinary Care Team (ICT) Meeting
- Field visits for members who may benefit from high-touch care coordination
- Ongoing care coordination and health promotion
- Coordination and support for members experiencing a transition of care
