PASC-SEIU Plan ID Card
Your Member ID Card has all the information you need to see your doctor, call the Member Services Department, or contact the Nurse Advice Line. It will also allow you to obtain your prescription medication.
Getting Your L.A. Care ID Card
You will receive an ID card that shows you are an L.A. Care member. Keep your member ID card with you at all times. Show the member ID card to the doctor, pharmacy, hospital or other health care provider when you seek care.
Never let anyone use your L.A. Care ID card. Letting someone else use your L.A. Care ID card with your knowledge is fraud.
Within two weeks of enrollment, you will receive a member ID card with the Primary Care Physician (PCP) name, medical group, and PCP phone number. You can choose to schedule an appointment with your assigned PCP or you can select another PCP by calling 1-844-854-7272 (TTY 711) or by accessing the member portal.
