L.A. Care Medicare Plus ID Card

L.A. Care Medicare Plus enrollees receive one Member ID Card, containing important information needed to access Medicare and Medi-Cal services covered by our plan, including long-term services and supports, certain behavioral health services, and prescriptions. You must show this card when you get any services or Part D prescriptions.

Getting Your L.A. Care ID Card

As a member of L.A. Care Medicare Plus, you will receive a Member ID Card when you first join our health plan and whenever we make changes to the appearance or content of the Member ID Card.
 

If your ID card is lost, stolen, or needs to be replaced or if you have changed your primary care provider or medical group, you can

  1. Get a new ID card mailed to you. Call Member Services at 1-833-LAC-DSNP (1-833-522-3767) TTY: 711 24 hours a day, 7 days a week, including holidays.
  2. Print an ID card from your L.A. Care Connect member account. You must have or create an account.
     

Understanding Your Member ID Card

Your Member ID Card contains important information. Below is an explanation of what you will find displayed on your Member ID Card.

L.A. Care Medicare Plus Plan: This Plan Name indicates that you are enrolled in the L.A. Care Medicare Plus (HMO D-SNP Plan) health insurance product.

L.A. Care Health Plan: This logo indicates that the L.A. Care Medicare Plus health insurance product is part of L.A Care.

Member Name: Your name will be displayed on the Member ID Card. Only the person indicated on the Member ID Card may use this card to obtain services.

Member ID:  This number is unique to you and can be used to create a member profile on L.A. Care Medicare Plus webpage by clicking the "Member Sign In" button. Once your profile has been created you can sign in to view your eligibility status, benefits and claims history. You can also change your doctor or medical group, request, view, and/or print your Member ID card, manage your medications and more.

PCP/Group Name: The primary care physician or group you selected or was assigned to you. Your primary care physician is associated with this medical group. You should visit your primary care physician for all health care needs. Your primary care physician will be able to refer you to a specialist depending on your medical needs.

PCP Phone: This is the number you can contact to reach your primary care provider. Call this number to make or change appointments with your doctor.

Copays: 

PCP/Specialist - The dollar amount or percentage that you will pay as your share of the cost when you visit your primary care physician or specialist. 

ER - The dollar amount or percentage that you will pay as your share of the cost when you receive care at the Emergency Department of a hospital. 

Rx Information: Your pharmacist needs this information to fill your pharmacy request.

Medicare Rx: This logo is a symbol of your Medicare drug coverage.

Member Services: This is the number you can call when you have questions or concerns about health care services or the coverage you get under your plan.  We can also provide general information such as provider availability, drug coverage, and health benefits covered under your plan.

Behavioral Health Services: This is the number you can contact regarding behavior health services such as behavioral, psychological and emotional health.

Dental Services: This is the number you can call if you need help finding a dentist in our network.

24-Hour Nurse Advice Line: This is the number you can call if you have questions regarding a non-emergency medical situation or concern. A nurse will be available 24 hours a day, 7 days a week to answer your questions and offer advice.

The portion of the card under the blue line is for your doctor.
