L.A. Care Medicare Plus ID Card
L.A. Care Medicare Plus enrollees receive one Member ID Card, containing important information needed to access Medicare and Medi-Cal services covered by our plan, including long-term services and supports, certain behavioral health services, and prescriptions. You must show this card when you get any services or Part D prescriptions.
Getting Your L.A. Care ID Card
As a member of L.A. Care Medicare Plus, you will receive a Member ID Card when you first join our health plan and whenever we make changes to the appearance or content of the Member ID Card.
If your ID card is lost, stolen, or needs to be replaced or if you have changed your primary care provider or medical group, you can
- Get a new ID card mailed to you. Call Member Services at 1-833-LAC-DSNP (1-833-522-3767) TTY: 711 24 hours a day, 7 days a week, including holidays.
- Print an ID card from your L.A. Care Connect member account. You must have or create an account.
