L.A. Care’s Care Transitional Care Services (TCS) Program is a program that helps members get resources after being in the hospital or nursing facility.
The TCS Program can help members:
- Make doctor appointments
- Get help with transportation
- Reduce emergency room use
- Manage medications
- Access community resources, such as food.
- Find caregiver support
- Navigate the healthcare system
Eligibility: All L.A. Care members with Medi-Cal are eligible to receive services if they have been admitted into a hospital or facility.
