Day Habilitation programs can be given in or out of your home. The program may help you learn or grow skills like self-care, working with others, and doing things on your own.
More skills you may learn to help you live better in your community are:
- How to use public transportation;
- How to deal with and solve problems;
- How to be a part of the community;
- How to build and grow relationships;
- How to cook, clean, and other daily living skills;
- How to shop and handle money;
- How to access local support such as police, fire, and other aids.
Other help you may receive is:
- Finding and moving into a home;
- Finding and choosing housemates;
- Finding home items and furniture;
- Taking care of landlord issues;
- Taking care of finances (money issues);
- Finding, teaching, and managing helpers that help you with daily tasks;
- Talking to government workers and offices;
- Protecting your rights;
- Building a support group;
- Connecting you to other health care and social services you may need that are available through L.A. Care, such as other Community Supports or Enhanced Care Management;
- Helping you find other housing aid if you do not qualify for Housing Transition/Navigation Services;
- Help with wages and programs like General Assistance/General Relief and SSI; and
- Working with your care team to get other services you may need such as healthcare, mental health support, and to help with drug use.
Eligibility
To join the program, you must be an active member with Medi-Cal. You must also have one of the following:
- You meet the definition of being homeless; OR
- You were homeless but found a home in the last 2 years; OR
- You meet the definition of being at risk of losing your home; OR
- You were at risk of having to stay in a place like a hospital or nursing facility.
Other restrictions may apply.
