The National Committee for Quality Assurance (NCQA) has recognized Herald Christian Health Center, a recently designated federally qualified health center (FQHC) in San Gabriel, The Children’s Clinic in Long Beach and Watts Health Center in South Los Angeles (both FQHCs) for achieving Patient-Centered Medical Home (PCMH) status. To receive recognition, standards had to be met in the following areas: enhanced access and continuity, identify and manage patient populations, plan and manage care, provide self-care support and community resources, track and coordinate care and measure and improve performance. These practices are a part of L.A. Care’s Patient-Centered Medical Home (PCMH) Initiative, which was launched in October 2010.
