San Fernando Valley Pediatric Medical Group Earns NCQA Patient Centered Medical Home Recognition

Congratulations to the Kids & Teens Medical Group in Mission Hills, which recently received National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) recognition. NCQA recognizes medical practices that provide patients with coordinated care across different settings. The PCMH model provides greater access, continuity, coordination and self-management of care.

The Kids & Teens Medical Group is a part of L.A. Care’s Patient Centered Medical Home (PCMH) Initiative, which launched in October 2010. We are proud to announce that, since L.A. Care launched the first cohort with 10 practices, two have received NCQA recognition.  Another four practices have submitted applications and are awaiting a response from NCQA. The second cohort, launched in May 2012, has seven practices, of which four have submitted applications, one has received recognition and the remaining three are also awaiting a response from NCQA. The participating practices serve approximately 100,000 L.A. Care members.
