Be ready! Your L.A. Care Medicare Advantage (HMO SNP) patients are receiving their Passport to Good Health, which may impact your office operations. The Passport includes a personalized report of 11 health measures and a letter instructing members to bring the report to their next appointment to verify and schedule needed lab tests, screenings or vaccinations. The Passport to Good Health is a way to empower members to take an active role in their well-being.
