About the Coordinated Care Initiative
Under California’s Coordinated Care Initiative, most Dual Eligibles will be able to join a health plan to receive coordinated Medi-Cal and Medicare benefits through the new Cal MediConnect program. Additionally, Long-Term Services and Supports (LTSS) will transition from a Medi-Cal Fee for Service benefit to a Medi-Cal health plan benefit, and Medi-Cal beneficiaries will join a health plan to receive these services.
About Cal MediConnect
Cal MediConnect is a three-year voluntary program for people with both Medicare and Medi-Cal. It benefits seniors and people with disabilities and will combine medical, behavioral, long-term and home and community-based services in one health plan. L.A. Care Health Plan is one of five well-respected health plans participating in Cal MediConnect in Los Angeles County.
