    For over 20 years, L.A. Care Health Plan has provided Angelenos access to quality health care. From our humble roots as a small, local insurance plan, we have grown to become the nation's largest publicly operated health insurer with 2.2 million members. One out of every five Angelenos is an L.A. Care member. If you ask me, that's a force to be reckoned with.

    Recently, under the strategic vision of Chief Executive Officer John Baackes, the plan has expanded its focus. Our mission has not changed — we still work tirelessly to provide access to quality health care for vulnerable and low-income communities, and to support the health care safety net that delivers that care. We have expanded our focus toward a new way of caring for members. Not only does the organization care about members' physical health, L.A. Care truly cares about the whole person. In addition to delivering quality health care, L.A. Care is now working to ensure members have safe housing, access to healthy foods, reliable transportation, stable income and early education opportunities to help ensure they are living their best lives.

    As evidence through the content to follow, you'll find a number of ways that L.A. Care has worked to improve the daily quality of life of our members. We are:

    • - Making significant investments to improve food security for members
       
    • - Forging new partnerships to identify homeless individuals who will be provided with housing and other resources
       
    • - Engaging in local, state and national advocacy efforts to preserve access to health care despite a tumultuous political climate

    I am proud to serve on L.A. Care's Board of Governors, and I am proud of the organization L.A. Care has become. This health plan has become a force for positive change in our community. I look forward to celebrating the success of these critical efforts in the future.

    Giving A Voice To Our Members

    Over the past year, L.A. Care Health Plan's role as an advocate has become more important than ever. Advocating on behalf of our members to ensure they have appropriate access to health care is not new for L.A. Care. But, given the current political climate, I have realized how critical it is that L.A. Care become the voice of its members at local, state and national levels on pressing issues that may have a lasting impact on low-income and vulnerable populations.

    With the Trump administration's ongoing threats to jeopardize affordable health care coverage, L.A. Care is more determined than ever to work with partners and coalitions to ensure lawmakers hear us. On behalf of our members and providers, L.A. Care will be vigilant and alert to legislation and regulatory proposals impacting Medicaid and the Affordable Care Act, which remains the law of the land. We will seek to oppose, modify or support changes to current regulation as necessary, depending on how such changes impact our members, providers or our ability to serve them.

    Doing this is just one of the ways we will continue to elevate health care in Los Angeles County. L.A. Care also is committed to expanding its reach in the community by:

    • - Adding more Family Resource Centers to the current five – two in the next year, with a goal of 13 total. Having a presence in the communities we serve allows our social workers and case managers to more intimately understand the needs of our members, offering immediate resources and access to care
       
    • - Partnering with organizations that focus on the daily needs of our members, like Project Angel Food, whose mission is to feed and nourish the sick as they battle critical illness
       
    • - Educating In-Home Supportive Services caregivers of Medicare-Medicaid beneficiaries about care management, continuity of care and enhanced communication. Real life skills, including CPR and infection control, are taught in a 10-week series that will help keep these members in the comfort of their homes for as long as possible

    As the political climate continues to present challenges, I reaffirm my commitment that L.A. Care will continue to be the voice of our more than two million members and the safety net providers who serve them.

    Improving the Daily Lives of Our Members

      L.A. Care believes it is our responsibility to address our members' whole health. This means not just being concerned about physical health, but access to healthy food and drinking water. It means having safe parks, good schools and a stable income. These “social determinants of health” influence your risk for disease and how long you live – and L.A. Care is committed to doing something about them.

      We have prioritized several major initiatives targeting social determinants of health. The first was a $20 million commitment over five years – the largest ever in L.A. Care's 20-year history – for Brilliant Corners and Los Angeles County's Housing for Health program. This initiative aims to secure permanent supportive housing for homeless individuals like Ma'Lissa Simon, whose story we share in the video.

      Our Community Health Investment Fund (CHIF) improves community health, expands health insurance coverage and strengthens the safety net, providing $66 million since 2001. Last year, CHIF increased the reach of L.A. County's Whole Person Care, a five-year pilot program that obtains federal matching dollars for support services to high-risk Medi-Cal beneficiaries.

      L.A. Care's Community Wellness Initiative provides $1 million in funding to organizations to help enroll individuals and families in CalFresh or Earned Income Tax Credit (EITC) programs. In the past year, grantees enrolled 2,774 families, consisting of over 7,700 individuals, into the CalFresh program. They also helped low-income residents prepare income tax returns and secure refunds totaling over $2.6 million, including over $850,000 in EITC credits.

      Lastly, L.A. Care created a partnership with several immigration rights organizations. Key components of this coalition are support of local “Know Your Rights” workshops and an extensive social media campaign, which reaches those who are too afraid to ask for help in person.

    • Supporting Caregivers

      In partnership with the California Long Term Care Education Center, L.A. Care trains In-Home Supportive Services caregivers of Medicare-Medicaid beneficiaries. The 10-week course strengthens care management, continuity of care and communication. The program has graduated 579 caregivers, with another 321 currently enrolled. A mid-point program survey showed that L.A. Care members reported improved physical (86%) and mental health (90%) since their caregivers went through the training.

    • Member Access To Resources

      To ensure our Medi-Cal members are receiving all of their eligible government benefits, L.A. Care partnered with Human Arc, an industry leader in helping patients enroll in government assistance programs. Human Arc helps L.A. Care members apply for critically-needed resources like Supplemental Security Income and Social Security Disability Income. This additional income will help feed, clothe and shelter these members and their families, who otherwise might not have had access to such important safety net benefits.

    • A National Voice for Members

      L.A. Care prides itself on being a voice for its members on local, state and national levels. Through leadership roles and active participation in trade associations, L.A. Care advocates for issues that affect the stability of health care coverage and access to services and resources. From signed coalition letters with health plans across the country, to meetings with legislative delegates, we ensure that the needs and concerns of L.A. Care's members are at the forefront of policy debates.

    • NCQA Accreditation

      L.A. Care is proud to be accredited by the National Committee for Quality Assurance (NCQA). The NCQA Health Plan Accreditation process evaluates how well a health plan manages quality throughout every part of our delivery system – physicians, hospitals, affiliated providers, and administrative services – to continuously improve health care for our members. This achievement validates that in addition to being the largest publicly operated health plan, L.A. Care is also among the best.

    • The Value Initiative for IPA Performance

      L.A. Care launched the Value Initiative for Independent Practice Association (IPA) Performance (VIIP). This first-of-its-kind initiative ranks IPAs on specific measures, including the Healthcare Effectiveness Data and Information Set (HEDIS), member satisfaction, access and availability, utilization and encounters. The VIIP program was then merged with L.A. Care's Pay-for-Performance (P4P) program to streamline reporting and tie-in annual P4P dollars to IPA performance. Last year, L.A. Care provided more than $12.5 million in VIIP/P4P incentives to encourage the provision of the best possible care to our members.

    • L.A. Care Direct Network, A Direct Partnership for All of L.A.

      L.A. Care first launched the L.A. Care Direct Network, A Direct Partnership for All of L.A. to improve access to care for its Medi-Cal members in the Antelope Valley. The network brought in 136 primary care physicians and specialists and the Antelope Valley Hospital, adding to our existing provider network in that region of more than 70 primary care physicians and 200 specialists. These providers who directly contract with L.A. Care receive a number of benefits, including streamlined patient care communication, expedited authorization of claims, and training resources. Last year, we expanded the network to South Los Angeles, Long Beach, East Los Angeles and Central Los Angeles, and plan to continue expanding the direct partnership in 2018.

    • Champions for Health Information Technology

      L.A. Care believes health information technology saves lives, improves health outcomes and should be available to all, especially L.A. County's safety net providers. In 2015, the California Department of Health Care Services awarded HITEC-LA, a project of L.A. Care, a contract of up to $10.8 million over three years to provide electronic health record (EHR) technical assistance to L.A. County providers. As part of this contract, HITEC-LA enrolled 2,165 Medi-Cal health care providers into the California Technical Assistance Program (CTAP), and works with them to use their EHR systems in a meaningful way.

    • Keeping in Touch with thePULSE

      L.A. Care publishes a monthly e-newsletter, thePULSE, for our provider partners. thePULSE features articles about health care, clinical protocols and new provider resources. With over 7,000 thePULSE subscribers, L.A. Care is dedicated to keeping providers informed and up-to-date, especially on legislative and regulatory proposals that may impact patients and clinical practice.

      L.A. Care pairs technology and health education to boost postpartum visits and care coordination.
       

    • Two Million+ Members Strong

      L.A. Care enrolled its two millionth member, a young working woman who needed affordable coverage.

      L.A. Care’s FRCs have been providing resources to Los Angeles County neighborhoods since October 2012.

      Our health promoters program is a recognized model for grassroots health education, reaching more than 39,000 community members.

      Thanks to our Consumer Advisory Councils, L.A. Care provided 26 clinics with adaptive equipment – 75 adjustable exam tables, 82 accessible scales and 77 listening devices.
      Community clinics were invited to view and purchase adaptive exam equipment using grant funds from L.A. Care.

      Food Security

      L.A. Care has invested over $11.2 million in safety net dental programs. As a top funder of oral health in Los Angeles County, we awarded $600,000 in grants last year.

      Giving Back to the Community

      Over the past year, L.A. Care employees supported the community through blood drives, food and toiletry donations and a number of volunteer opportunities, including Union Rescue Mission, Project Angel Food and the annual Homeless Count.

      Celebrating Inclusion

      In May 2017, L.A. Care proudly joined tens of thousands of people marching in the Long Beach Pride Parade. Celebrating diversity and inclusion, L.A. Care employees supported the LGBT community and the bridge-building activities that educate and encourage acceptance and love.

      Cross Functional Training

      L.A. Care offers its employees many opportunities to further their education and skills. This past year, learning opportunities included a partnership with California State University, Los Angeles, tuition reimbursement of up to $7,500 and Customer Service Week (a Call Center shadowing program). Employees also participated in town hall meetings with the CEO, and held activities and panels during national awareness weeks and months.
