Special Notice: Beginning April 1, 2010, Health Integrated, an L.A. Care partner, will provide the Synergy Targeted Population Management® pilot program (“Synergy") to a number of L.A. Care members in our direct Medi-Cal line of business. The Synergy program will identify eligible members and develop interventions that will assist the member in reaching a treatment plan and medication adherence goals. For Medi-Cal Patients:
Specialized mental health services are provided through the Los Angeles County Department of Mental Health (LACDMH). Services from LACDMH can be provided with or without a referral.
LACDMH may be reached toll-free at 1-800-854-7771.
L.A. Care will coordinate and cover laboratory, radiological and radioisotope services needed for the diagnosis, treatment and monitoring of a mental health condition.
For Healthy Families, Healthy Kids and L.A. Care Health Plan Medicare Advantage HMO Plan Patients:
L.A. Care contracts with PacifiCare Behavioral Health (PBH) to provide inpatient and outpatient mental health services for these members. This includes drug and alcohol abuse services.

Special Note:
Care for members determined to have a serious emotional disturbance (SED) condition will be provided by the county mental health department. The member will remain enrolled in L.A. Care and will continue to receive medical care from plan providers for services not related to the SED condition.
