Pharmacy and Therapeutics Committee Updates

The L.A. Care Pharmacy and Therapeutics Committee, comprised of practicing physicians and pharmacists, meets quarterly to consider additions to and deletions from the formulary based on new information or new drugs that become available.

If you would like to request a drug that requires a Prior Authorization (PA) or is not on the formulary, please fill out a Medication Request Form and fax it to MedImpact at (858) 578-9732 or call MedImpact Directly at (800) 788-2949 and provide all necessary information requested.

L.A. Care highly encourages our network physicians and pharmacists to provide suggestions and comments for formulary additions and changes.
