Ongoing Opportunities:
- Hospital Readmissions Reduction Webinar: As part of its hospital readmissions initiative, L.A. Care’s contracted hospitals were invited to participate in a webinar on readmissions reduction. The webinar included an overview and results of the Better Outcomes for Older Adults through Safe Transitions (BOOST), in which 7 hospitals in L.A. Care's network participated. Hospitals and providers are encouraged to participate in this ongoing collaborative, which will involve future webinars. Contact the QI department for more information.
