Healthy Kids Program

L.A. Care administers the Healthy Kids program, which provides health coverage to children up to age 18. The program is offered by L.A. Care Health Plan and is sponsored by First 5 LA and the Children's Health Initiative of Greater Los Angeles.

The medications listed on this formulary are subject to change pursuant to the formulary management activities of L.A. Care Health Plan (L.A. Care) and its Plan Partners, and approval by applicable regulatory agencies. The presence of a medication on this formulary does not guarantee that a member will be prescribed that drug by his or her primary care physician or contracting provider for a particular medical condition. If a generic medication is available, it must be dispensed in place of the brand name medication. Members should consult their Evidence of Coverage or contact the Plan Partner Member Services Line for additional information. Brand name medications are the registered trademarks of their manufacturers.

L.A. Care Health Plan, in cooperation with our Plan Partners, has designed a uniform Prior Authorization Form. This form is accepted by all Plan Partners and should increase provider and member satisfaction while streamlining administration.

 Healthy Kids covers children: 

 Up to their 19th birthday 
Who are Los Angeles County residents, U.S. citizens, nationals, eligible qualified immigrants, or undocumented persons who meet all other eligibility requirements 
 In families whose incomes are at or below 300% of the Federal Income Guidelines 
Who are not eligible for no-cost full-scope Medi-Cal, Healthy Families, and Access for Infants and Mothers (AIM) 
 Who have not been covered by private (employer-sponsored) health insurance in the last three months
