Pediatric Weight Management
Toolkit
L.A. Care Health Plan is pleased to provide your office
with this provider toolkit as part of our Obesity Prevention and Treatment
Initiative to help screen, prevent, and manage children at risk for
overweight and already overweight in your practice. The
kit includes: CDC growth charts, AAP weight management decision tree,
workup algorithms for selected co-morbidities, counseling tips, patient
education sample pamphlet and wall poster, and BMI wheel. To order

Individual Health Education Behavioral
Assessment (IHEBA)
The IHEBA
form, also known as the "Staying Healthy" Assessment Tool, is a behavioral
assessment required to be completed for all L.A. Care members at
the Initial Health Assessment (IHA) and periodically thereafter as
the patient ages. This form will help you and your
office:

Identify high risk behaviors in your
patients -
Provide health education interventions including
referrals, materials and counseling -
Improve your patients' health and
well-being
The "Staying
Healthy" Behavioral Assessment Tool is available in five
age-appropriate categories in English, Spanish, Chinese, Lao, Hmong, Russian and
Vietnamese. Additional educational resources called
"Staying Healthy" California Tip Sheets
are also available.
