Learn How To Stay Healthy

Health education can help you and your family stay healthy and reduce the chance of having major health problems. Visit our health education materials page  to view and print information about how to manage your health.  

 

Check-Ups and Immunizations (shots)
It is very important for you and your family to go to the doctor for check-ups and immunizations (shots). Preventing illness early helps you and your family live a healthier life.  Children younger than 18 months should receive their first health check-up within 60 days of joining L.A. Care Health Plan. Children 18 months and older should get their first check-up within 120 days. After that, go to the doctor for regular visits.  Follow the child and adult schedules to know when to go to the doctor.  At some of these visits, you and/or your child will get needed immunizations (shots).  Always bring your immunization card to your child's doctor to fill out. You may need to show proof of immunization to enroll your child in public or private school, day care centers or other programs.

"Staying Healthy" Assessment Tool
At the first health check up, you will be asked to fill out a form called the Individual Health Education Behavioral Assessment (IHEBA) or "Staying Healthy" Assessment Tool for yourself and/or your child. This form asks about lifestyle, behavior, home, culture, and language needs. It is available in English, Spanish, Chinese, Lao, Hmong, Russian, and Vietnamese for five age groups. Answer the questions as best as you can. This will help your doctor find any problems and make a plan to fix them. Your doctor may refer you to health education services such as group classes, one-on-one couseling or provide you with health education materials. You can print the
"Staying Healthy" Assessment Tools and "Staying Healthy" Tip sheets and bring them with you to your next doctor appointment.
