HealthCare Partners Signs Agreement With L.A. Care For Los Angeles Area Patients Enrolled In Cal Mediconnect

HealthCare Partners today announced it has signed a provider agreement with L.A. Care Health Plan to provide services for patients eligible for both Medicare and Medi-Cal (Medi-Medis) in the new program known as Cal MediConnect.

TORRANCE,CA – L.A. Care is one of five health plans in Los Angeles County participating in Cal MediConnect, a three-year Medicare and Medi-Cal program that integrates medical care, long-term care, mental health, substance abuse programs, and social services under a coordinated care plan.

HealthCare Partners will manage all medical care for beneficiaries in this program while also working with L.A. Care to actively participate in the management of long-term services and supports. Extensive resources are also being provided by both organizations to fully coordinate the behavioral health needs for this population.

The agreement is subject to final approval by the Department of Managed Health Care (DHMC).
