IMPORTANT PLAN CHANGES EFFECTIVE JULY 1, 2010!
On July 1, 2010, there will be changes to the Healthy Kids Program for children ages 6-18 (Healthy Kids 6-18 Program). Letters explaining the changes were sent to families with children in the Healthy Kids 6-18 Program on May 21, 2010. The letter also included a Premium Increase Response Form that families must complete and return by June 21, 2010.
