Description:
The Robert E. Tranquada Awards are intended to help small community clinics in Los Angeles County to strengthen their clinical, operational, revenue streams and health information technology infrastructure for long-term impact.

Description: Program designed to help community-based clinics reduce wait times for and at appointments, increase provider panel sizes and reduce cost per visit.

Description: Services to Medi-Cal beneficiaries over the age of 21 who lost their dental benefits as of July 1, 2009.

Robert E. Tranquada Safety Net II – 2008-2009
Description: Assessment and planning of long-lasting impact infrastructure improvement projects for small community-based clinics. The second phase will support implementation of assessment and planning projects.

Health Information Technology (HIT) I -- 2007-2008
Description: Acquisition of health information technology, staff training and implementation of technology. Initiative is aligned with the second phase of Building Clinic Capacity for Quailty (BCCQ), a health IT learning collaborative.

Description: Forty-eight community-based clinic sites procured adaptive equipment such as exam tables and weight scales to improve accessibility.

Robert E. Tranquada Safety Net I -- 2006-2007
Description: Strategic infrastructure projects that have a lasting impact, including upgrading or acquiring information technology, purchasing medical equipment, remodeling to accommodate new capacity, health information technology training and hiring of strategic consultants to help with long-term infrastructure planning.

Seniors and Persons with Disabilities (SPD) Initiative
Seniors and Persons with Disabilities I -- 2005-2006
Description: Initiative has three components:

1. Safety Net Clinics: Improve physical and programmatic access, including availability of accessible equipment such as exam tables, weight scales and hearing devices.

2. Community-Based Health and Wellness Programs: Provide health and wellness education on topics such as nutrition, physical fitness and healthy lifestyle.
3. Promotores Program: Train Spanish-speaking health promoters to assist seniors and persons with disabilities in accessing health and wellness services.

Description: Pilot program to assess community-based asthma self-management programs for children ages five to 18 and to promote effective models of for expansion and replication.
